Carers

Please let us know if you are a carer (that is if you look after someone who could not get by without your support) or you have a carer, then please let the practice know so that we can give you some additional support.

Please complete a Carer Registration form by telephoning the surgery and ask for Theresa, or ask at our reception for further details.

Mental Health Help for Young People

Confidential support is available for all young persons, upto 25 years old, from the Eclypse services team.

Self harm support for parents and carers guide

This guide was developed from talking to parents and carers of young people and is aimed at helping parents, carers, other family members and friends cope when a young person is self-harming. It includes information on the nature and causes of self-harm, how to support a young person when facing this problem and what help is available.

Transgender

Please make sure that your GP knows about your gender status so that we can ensure that you get the best care, and can participate in appropriate screening programmes.

Change Your Weight

Salford Weight Management programme

Do you or someone you know want to lose weight and improve their health?

The Salford Active Lifestyles team offer a free 12 week programme that will provide you with:

  • Individual nutrition advice
  • Physical activity
  • Tips and techniques to keep you motivated
